Getting started
How home health care begins
Most patients start with a referral from their doctor or hospital. Families can also call us first and we'll help with the rest.
Referral
A physician, hospital, or family member contacts our office by phone or fax.
Intake call
We confirm the doctor's orders, check insurance, and schedule the first visit.
First nurse visit
A nurse visits the home, does an assessment, and builds the care plan.
Ongoing care
Regular visits continue, and we keep the physician informed of progress.

Fax the referral and our intake team will call to confirm. For urgent starts of care, call the office after faxing.
